However, it is necessary to verify that the minor is properly registered with AIRE through FASTIT and that the data are correct.<\/p>\n<p><strong>IMPORTANT: <\/strong>Read the information regarding parental consent signature (*)<\/p>\n<p><strong>OPTION 1<\/strong>, by post it is possible to send to the Consulate:<\/p>\n<ul>\n<li><strong><u>the passport application form<\/u><\/strong>, duly completed and signed (*) by both parents;<\/li>\n<li><strong>a copy of both parents\u2019 passports<\/strong>, showing their personal data, their signature, and the issuing Authority; if they do not hold a valid passport, a valid identification document clearly showing photo and signature (e.g. Canadian provincial driver\u2019s licence);<\/li>\n<li><strong>the minor\u2019s passport<\/strong>, if the minor holds a still valid Italian passport; otherwise, a copy of the <strong>expired Italian passport<\/strong> with personal data and issuing Authority; if this is the first Italian passport application, a copy of the minor\u2019s valid identification document;<\/li>\n<li><strong>two (2) recent identical photographs<\/strong> (maximum 6 months), in Canadian passport format (5 x 7 cm), with front-facing and centered face, neutral expression;<\/li>\n<li><strong>the exact amount<\/strong> \u2014 exclusively in Canadian currency \u2014 for the payment relating to the issuance of the passport, in cash, money order or bank draft; for the updated quarterly fee, consult <u>the consular fee<\/u> table, section 3, under \u201cpassport\u201d;<\/li>\n<li><u>the release form<\/u> duly completed and signed;<\/li>\n<li><strong>one (1) prepaid and pre-addressed envelope<\/strong>; it is recommended to use a service that provides tracking (for example Xpresspost, FedEx, UPS, etc.);<\/li>\n<\/ul>\n<p><strong>Note:<\/strong> if the application concerns more than one minor under the age of 12, send only one envelope and one Money Order\/Bank Draft with the total amount.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>OPTION 2, if one of the parents\/legal guardians has an appointment for passport renewal. <\/strong>It is possible, upon request via email and by indicating it in the NOTES field of Prenot@Mi, to obtain the passport also for the minor.<\/p>\n<p>The request email must include:<\/p>\n<ol>\n<li><strong><u>the passport application form<\/u><\/strong>, duly completed and signed (*) by both parents;<\/li>\n<li><strong>both parents\u2019 passports<\/strong>, showing their personal data, their signature, and the issuing Authority; if they do not hold a valid passport, <strong>a valid identification document<\/strong> clearly showing photo and signature (e.g. Canadian provincial driver\u2019s licence);<\/li>\n<li><strong>the minor\u2019s passport<\/strong>, if the minor holds a still valid Italian passport;
