The appointment for the issuance of the passport of a minor must be booked through the Prenot@Mi portal using the account of one of the parents, entering the name of the minor in the notes field (specify name, surname, place and date of birth). Parents are invited to verify that the minor is regularly registered with AIRE through FASTIT and that the data are correct.
N.B.: When booking an appointment on Prenot@Mi, please make sure to upload the duly completed and signed passport application form in the mandatory “other” field.
Kindly note that appointments without the required form attached will unfortunately have to be cancelled.
Once the appointment is booked, it must always be confirmed via Prenot@Mi between the tenth and the fourth day before the scheduled date; otherwise, it will be automatically cancelled.
* Parental consent signature:
- If the parents are Italian or European Union (EU) citizens, they may sign the consent declaration present in the passport application form.
- If one of the two parents is a non-EU citizen, they must affix their signature: 1. in person with us, at the Consulate General, if residing in this jurisdiction (the signature will be affixed on the day of the appointment); 2. at the competent Police Headquarters or Police Station, if residing in Italy; 3.at the competent Consular Office, if residing in another jurisdiction or in another Country.
On the day of the appointment it will be necessary to present:
- the original of the passport application form;
- the copy of the passports or identity documents of the parents;
- (or a valid identification document, if requesting the issuance of the passport for the first time), where the personal data and the issuing Authority appear;
- two recent identical photographs (maximum 6 months), Canadian passport format, face front-facing and centered, neutral expression;
- the exact amount — exclusively in Canadian currency — for the payment relating to the issuance of the passport, in cash, money order or bank draft; for the updated quarterly fee, consult the consular table, section 3, under “passport”.
