Passport request for minors aged between 12 and 18 resident in this consular district
The passport is issued by appointment only through Prenot@mi and is aimed at acquiring the minor’s fingerprints and signature. Please note that if any of the documentation listed below is missing, it will NOT be possible to issue the passport on the day of the appointment.
Immediately after setting the appointment, you must send to montreal.passaporti@esteri.it:
- the passport application form, duly completed and signed by both parents;
- a copy of the passport of the minor (or a valid identification document, if the passport is being issued for the first time), where the personal data and the issuing authority appear;
- a copy of the passports of both parents, showing their personal data, their signature and the issuing authority. The total size of the attachments must not exceed 5 MB.
Regarding the signature of consent:
- If the parents are Italian citizens or citizens of the European Union (EU), they can sign the consent form in the application form.
- If one of the two parents is a non-EU citizen, he/she must sign: in our presence, at the Consulate General, if he/she resides in this district (the signature will be affixed on the day of the appointment); or at the Police Headquarters or the competent Commissariat, if he/she resides in Italy; or at the competent consular office, if he/she resides in another constituency or in another country.
On the day of the appointment, the minor must bring:
- the original of the passport application form sent by e-mail.
- copies of the parents’ passports or identity documents, sent by e-mail.
- his/her current passport, if any.
- two recent identical photographs (max 6 months) of Canadian passport format, with frontal and centered face and neutral expression.
- the exact amount – exclusively in Canadian funds- for the payment related to the issuance of the passport, in cash, money order, or bank draft; the updated quarterly fee can be found in the consular table, section 3, under ‘passport’.
For minors residing in the provinces of New Brunswick, Nova Scotia, Prince Edward Island, Newfoundland and Labrador who are registered with AIRE, an appointment for the acquisition of fingerprints and signature can be booked with the Honorary Consul Marilisa Benigno, thereby avoiding having to make a trip to Montreal.
