For minors aged 0 to 11, the passport has a shorter validity, and the collection of biometric data is not required. However, it is necessary to verify that the minor is properly registered with AIRE through FASTIT and that the data are correct.
IMPORTANT: Read the information regarding parental consent signature (*)
OPTION 1, by post it is possible to send to the Consulate:
- the passport application form, duly completed and signed (*) by both parents;
- a copy of both parents’ passports, showing their personal data, their signature, and the issuing Authority; if they do not hold a valid passport, a valid identification document clearly showing photo and signature (e.g. Canadian provincial driver’s licence);
- the minor’s passport, if the minor holds a still valid Italian passport; otherwise, a copy of the expired Italian passport with personal data and issuing Authority; if this is the first Italian passport application, a copy of the minor’s valid identification document;
- two (2) recent identical photographs (maximum 6 months), in Canadian passport format (5 x 7 cm), with front-facing and centered face, neutral expression;
- the exact amount — exclusively in Canadian currency — for the payment relating to the issuance of the passport, in cash, money order or bank draft; for the updated quarterly fee, consult the consular fee table, section 3, under “passport”;
- the release form duly completed and signed;
- one (1) prepaid and pre-addressed envelope; it is recommended to use a service that provides tracking (for example Xpresspost, FedEx, UPS, etc.);
Note: if the application concerns more than one minor under the age of 12, send only one envelope and one Money Order/Bank Draft with the total amount.
OPTION 2, if one of the parents/legal guardians has an appointment for passport renewal. It is possible, upon request via email and by indicating it in the NOTES field of Prenot@Mi, to obtain the passport also for the minor.
The request email must include:
- the passport application form, duly completed and signed (*) by both parents;
- both parents’ passports, showing their personal data, their signature, and the issuing Authority; if they do not hold a valid passport, a valid identification document clearly showing photo and signature (e.g. Canadian provincial driver’s licence);
- the minor’s passport, if the minor holds a still valid Italian passport; otherwise, a copy of the expired Italian passport with personal data and issuing Authority; if this is the first Italian passport application, a copy of the minor’s valid identification document.
At the appointment it will be necessary to present:
- the original passport application form sent by email, duly completed and signed (*) by both parents;
- both parents’ passports (or valid identification document);
- the minor’s passport (or valid identification document);
- two recent identical photographs (maximum 6 months), in Canadian passport format (5 x 7 cm), with front-facing and centered face, neutral expression;
- the exact amount — exclusively in Canadian currency — for the payment relating to the issuance of the passport, in cash, money order or bank draft; for the updated quarterly fee, consult the consular fee table, section 3, under “passport”.
* Parental consent signature:
- If the parents are Italian or European Union (EU) citizens, they may sign the consent declaration present in the passport application form.
- If one of the two parents is a non-EU citizen, they must affix their signature: 1. in person with us, at the Consulate General, if residing in this jurisdiction (the signature will be affixed on the day of the appointment); 2. at the competent Police Headquarters or Police Station, if residing in Italy; 3.at the competent Consular Office, if residing in another jurisdiction or in another Country.
