Minors under the age of 12 residing in this consular district

IMPORTANT NOTICE: Before proceeding, please read carefully the information provided here.

The acquisition of fingerprints and signature, concerns only passport applicants who have already reached the age of 12.

Option 1, by mail:

  1. the passport application form, duly completed and signed by both parents (*);
  2. copy of the passports of both parents, showing their personal data, their signature and the issuing authority;
  3. the child’s actual passport, if the minor already has an Italian passport and it is still valid; otherwise a copy of his/her expired Italian passport where his/her personal data and the issuing authority appear; if this is his/her first Italian passport application, a copy of the child’s valid ID;
  4. two (2) recent identical photographs (max 6 months) of Canadian passport format, with frontal and centered face and neutral expression;
  5. prepaid and self-addressed envelope; it is recommended to use a service that offers tracking (e.g. Xpresspost, FedEx, UPS etc.)
  6. the exact amount – exclusively in Canadian funds- for the payment related to the issuance of the passport, in cash, money order, or bank draft; the updated quarterly fee can be found in the consular table, section 3, under ‘passport’;
  7. the release form duly completed and signed.

Note: in case the application is for more than one child under 12 years of age, send only one envelope and one Money Order/Bank Draft with the total amount.

Option 2, by-in person appointment (with Prenot@mi):

  1. the passport application form, duly completed and signed by both parents (*);
  2. copy of the passports of both parents, showing their personal data, their signature and the issuing authority;
  3. the child’s actual passport, if the minor already has an Italian passport and it is still valid; otherwise a copy of his/her expired Italian passport where his/her personal data and the issuing authority appear; if this is his/her first Italian passport application, a copy of the child’s valid ID;
  4. two (2) recent identical photographs (max 6 months) of Canadian passport format, with frontal and centered face and neutral expression;
  5. the exact amount – exclusively in Canadian funds- for the payment related to the issuance of the passport, in cash, money order, or bank draft; the updated quarterly fee can be found in the consular table, section 3, under ‘passport’.

* Regarding the signature of consent:

  1. If both parents are Italian citizens or citizens of the European Union (EU), they can sign the consent in the application form (both signatures must be on the original form, no copies); if one of the Italian (or EU) parents resides outside of this consular district, they must sign the consent form at the competent consular office or Questura;
  2. If one or both parents are a non-EU citizens, they must sign the application (or the consent form):
    • at the Consulate General -by appointment, for people residing in our consular district and living close to Montreal;
    • in front of our Honorary Consul, for people residing closer to Quebec or Halifax;
    • at the Questura, if resident in Italy;
    • at the competent Consular Office, if the parent resides in another consular district (e.g. Vancouver) or in another country.
